As a Support Coordinator, you will provide support and information to our members, as you connect them to services that holistically support their wellbeing. Managing a caseload, you will maintain regular contact, to identify needs and ensure that individuals and families have access to the assistance and resources they need during challenging times. You will be part of a dedicated team of passionate, empathetic Support Coordinators based around Aotearoa New Zealand - this role is for 20 hours a week, and can be based from home in Auckland.
About us … Sweet Louise has led the support of those living with incurable breast cancer in Aotearoa New Zealand for 20 years. We provide practical, emotional and social support to our members, to help them live as well as possible for as long as possible. Our role is to support, not to provide medical assistance, or advice. About the role … - Manage a caseload of members and maintain regular contact to ensure they feel supported, and listened to.
- Provide support and information to meet the holistic wellbeing needs of our members
- Organise and facilitate meetings and events in your region.
- Build and maintain effective relationships with healthcare professionals, agencies and networks relevant to a range of breast cancer services.
- Maintain accurate and up to date records and confidential file notes in our systems.
About you … - Passionate about making a difference in the community and committed to our values.
- Aware of the unique challenges faced by the families we support, and able to respond to them with empathy and understanding.
- Adaptable, flexible and resilient, you are comfortable with an unpredictable landscape and have the ability to navigate complexity.
- Sound knowledge of the health system and the role of community services and facilities
